Why This Doesn't Exist in Canadian Healthcare

How the Health Story Works in Practice

1. Four Years of Results, Finally in One Place
A new member had completed annual bloodwork through his previous family practice for four years, but those results lived in a system he couldn't access. A cardiology referral from two years earlier had come and gone — results faxed back to the referring office, never discussed with him.

The results were delivered as a PDF, reviewed once over a phone call, and never referenced again. Thousands of dollars of data sitting in their inbox.
His Care Advocate requested the full report, underlying lab data, and imaging files. Once in his Health Story, his Stem Health physician could review the baseline in detail — cholesterol panel, inflammatory markers, calcium score, body composition — and identify which numbers warranted monitoring immediately.
Instead of repeating tests he'd already paid for, his physician built a care plan from real data on day one. When he completes his first Stem Health Annual Health Assessment later this year, the comparison won't start from zero — it'll pick up where his last assessment left off.
3. A Referral That Didn't Disappear
During an appointment, imaging flagged an endocrinology follow-up. In a traditional practice, this is where things stall: a fax goes out, the member waits, attends weeks later, and hopes the results find their way back to their family doctor.
The client's Care Advocate coordinated the referral, confirmed the endocrinologist received the relevant imaging and bloodwork in advance, and followed up to obtain the specialist's report. Findings, recommendations, and follow-up timeline were in his Health Story within 48 hours.
When his physician reviewed the update, the specialist's assessment was already alongside the original imaging and baseline thyroid panel. She added the monitoring recommendation to his care plan and set a six-month reminder. Nothing fell through.
