Stem Health offers two individual memberships - Foundation and Signature. Both begin with the same promise: a dedicated physician, a connected Health Story and a care team that remains involved between visits.
The difference is depth. Signature includes everything in Foundation, with greater access and more extensive diagnostics. The right choice begins with understanding what stays the same — and what each tier adds.
Foundation: Proactive care that stays connected
Foundation is designed for people who want primary care to feel consistent, personal and easier to navigate.
Membership includes:
● A dedicated physician - Your relationship begins with a 45-minute intake, giving your physician time to understand your history, medications, concerns and goals.
● A Health Plan - Created with you and revisited at each check-in, the plan brings your priorities and next steps into focus.
● Same-day or next-day appointments - Meet with your physician in person, by video or by phone when something comes up.
● A dedicated Care Advocate - They arrange referrals, follow up on results and keep communication moving between everyone involved in your care.
● Your Health Story - Visits, results and plans are held together in one clear record, allowing every conversation to build on the last.
● An annual biomarker panel - Your physician reviews markers across six areas of health and explains what the findings mean for you.
● Quarterly check-ins - Regular conversations give you and your physician time to review your progress and adjust the plan as your needs change.
● Support between visits - Membership includes after-hours nurse support, prescription renewals and delivery, care coordination and medical second-opinion coordination in Canada and the United States.
● Preferred pricing - Members receive preferred pricing on health assessments and diagnostics.
● Two sports medicine sessions each year -These sessions offer additional support for movement, recovery and physical health.
The Foundation plan creates a steady rhythm of care. The same physician knows your story, your care team keeps track of the details and your plan continues moving between appointments.
Signature: Care with greater reach
Signature begins with everything included in Foundation. It then adds more ways for care to meet you where you are.
Signature members also receive:
● At-home or office visits - Your physician can come to you up to four times each year, offering greater flexibility when travelling to the clinic is difficult or inconvenient.
● A direct line to your physician - You can reach your doctor directly when a question calls for their personal attention.
● A dedicated nurse consultant - Your nurse knows your file and can support clinical questions that arise between physician visits.
● Broader diagnostics - Pharmacogenetic testing can offer insight into how your body may process certain medications. Proactive genetic screening and counselling can help you and your physician consider inherited health risks with greater context.
● Six sports medicine sessions each year - Signature includes four more sessions than Foundation, allowing for more consistent support throughout the year.
The Signature plan offers a closer level of access and a deeper view of your health. It may feel right if you value care that can come to you, direct access to your physician or more extensive diagnostics from the beginning.

Some of the medical expenses connected with your care may qualify for the Medical Expense Tax Credit. Eligibility depends on the service and how it is billed, so a conversation with your accountant can help clarify what applies to you. You may also review the Canada Revenue Agency’s guidance on eligible medical expenses.
Choosing what feels right
Neither tier represents better care. Both are built around the same physician relationship, continuity and follow-through. The choice is about which added services you expect to use and value.
Foundation may suit you if your priority is having a physician who knows you, timely appointments, proactive check-ins and a care team that handles the coordination around your care.
Signature may feel more natural if you expect to use home or office visits, want a direct line to your physician or value having genetic screening, pharmacogenetic testing and additional sports medicine sessions included from the beginning.
Choosing Foundation does not close the door on deeper support. You can upgrade to Signature as your needs evolve, while genetic screening and genetic testing remain available as additional services. This gives you room to choose the care that feels right today without having to anticipate every future need.
Whichever plan you choose, the relationship at the centre remains the same. Your physician takes time to know you. Your care team stays close. And each part of your care builds on what came before.
